Healthy Communities, Healthy People Program

Healthy Communities Healthy People (HCHP) Program (used to be known as Step Up Florida) provides funding to all 67 Florida County Health Departments, including Nassau County, to develop and implement cardiovascular prevention programs addressing Healthy People 2020 objectives including nutrition, physical activity, cancer, heart disease and stroke, and diabetes.

The Nassau Healthy Communities Healthy People Program is dedicated to partnering with the local community, worksites, churches, schools, and private organizations to aid in development of policy and environmental changes that promote healthy lifestyles.
 


Current Programs Available:

Healthy Lifestyles for People with Disabilities: A nutrition and health education program geared towards individuals with mental and physical disabilities. Classes focus on eating healthy, general health, social well-being, and physical activity. Will soon be available at the local ARC.

Diabetes Self-Management Classes:
For anyone with, at risk for, or caring for an individual with diabetes. This is a four part series offered over 4 weeks for 2 hours one night/week. Classes focus on carb counting, general overview of diabetes including facts and myths, blood glucose monitoring/A1c and long-term complications. Fees for this program are as follows: $10 for Nassau County School District employees and Local Nassau county government employees; $20 for public; and free to LIP patients

Healthy Lifestyles Class:
A nutrition and health education class designed for Nassau County School Board employees as part of the Worksite Wellness Grant. Effective 2010 remainder openings will be advertised to the public for a charge. This is a 4 week program that meets one night a week for 2 hours. Eating healthy, heart health education, weight control and other health issues are covered in this class.

Healthy Cooking Class: A nutrition and health education class designed for Nassau County School Board employees as part of the Worksite Wellness Grant. Effective 2010 remainder openings will be advertised to the public for a charge. This is a 4 week program that meets one night a week for 2 hours. Healthy meals are prepared and class participants learn how to follow recipes, health benefits of the foods, and how to make healthy recipe substitutions.

MCLC Childhood Obesity Collaborative: A multi state collaborative to reduce childhood obesity funded by the Robert Wood Johnson Foundation. Only 10 counties throughout the state were selected by the DOH Office of Performance Improvement. In Nassau county Callahan Elementary school is our pilot school. Peaceful Playground stencils were purchased and painted in the bus loop area. One goal of this project is for children to increase physical activity. Soon we will be starting the final observational study to this project with the help of parent volunteers. As this project continues, we are looking to implement the stencils in other local schools.

Community Garden at YFSS: A community garden is hopefully coming to the YFSS. Details on how this garden will be used will be announced once decided.
